  • Graduate of a physical therapy curriculum approved by the American Physical Therapy Association.
  • Licensed by the State of Michigan.
  • One year of clinical experience preferred.
  • Physically move, position, and transfer clients up to 200# utilizing good body mechanics, lifting techniques and/or assistive devices.
  • Communicate effectively in verbal and written English and by telephone.
  • Possess current Michigan motor vehicle license, ability to drive a car, and insured vehicle capable of transport to patient homes or other offices under various weather conditions.
  • Possesses a working landline phone in the home.
  • Meets MHH established requirements for proof of health status.
  • CPR certification required.


Reports to appropriate clinical supervisor.

Collaborates with physicians, Home Health staff, community personnel as needed, and the patient/family to establish and implement patient care plan.

May work as a member of the Hospice team to help meet the needs of the terminally ill patient and his/her family.

  5. Meets established productivity standards.
  6. Complies with standard precautions, infection control, and safety procedures.
  7. Services provided by a physical or occupation therapist include initial and ongoing assessments to determine level of functioning including OASIS assessments at appropriate time points.
  8. Performs initial and interim assessment and evaluation of the client’s level of functioning.
  9. Problem identification and goal setting.
  10. Develops the plan of treatment in consultation with the physician.
  11. Provides physical therapy in accordance with the plan of treatment.
  12. Revises the plan of treatment as needed.
  13. Interdisciplinary coordination and adherence to the plan of care.
  14. Provides patient/family education as appropriate.
  15. Requisitions and manages equipment and supplies required to increase the client’s function and independence as appropriate to care.
  16. Records in a timely manner evaluation data, treatments, client’s response the therapy interventions.
  17. Supervises the physical therapy assistant (PTA) and paraprofessionals as required by policy and state regulations.
  18. Participates in quality and performance improvement measures.
  19. Participates in the inservice programs.
  20. Maintains CPR certification.
  21. May be expected to conduct peer review including site visits and chart review to document competencies and performance evaluation.
  22. Discharge planning and evaluation.
  23. Responsible for such other job duties as may be assigned by the manager.

  1. Sets goals related to the needs of the hospice patient.
  2. Consults with IDT members.
  3. Provides equipment required to increase the patient’s function and independence.
  4. Provides pain management, planning and evaluation.
  5. Provides specialized hospice training.
  6. Discharge planning and evaluation.
