SUMMARY
Munson Healthcare is seeking an outstanding individual to join their Family Practice team.

ENTRY REQUIREMENTS
- Master's degree and license in Social Work preferred with 3 years or 6000 hours related work experience.
- Licensed in good standing in the State of Michigan as a Mental Health Professional
- Minimum 2 years mental health experience. Prefer experience with co-occurring disorders (mental health and substance use disorders).
- ICRC-certified or certifiable chemical dependency counselor and/or licensed preferred.
- Demonstrated knowledge and proficiency in the treatment of Mental Health Disorders.
- Demonstrated knowledge and proficiency in substance use dependency treatment concepts and applications with focus on evidence based practices.

Reports to Munson Family Practice Physician.

SPECIFIC DUTIES

- Has basic understanding of Relationship-Based Care (RBC) principles, meets expectations outlined in Commitment To My Co-workers, and supports RBC unit action plans.
- Uses effective customer service/interpersonal skills at all times.
- Provides individualized treatment to patients and their families through assessment, diagnosis, planning, education, intervention, discharge planning and evaluation of outcomes.
- Conducts didactic and treatment group and individual sessions.
- Demonstrates competency through the annual competency assessment, customer evaluation, clinical staff dialogue, performance appraisals and staff education.
- Documents application of the treatment process focusing on outcome criteria in accordance with hospital, departmental and regulatory agency requirements.
- Communicates with other members of the health care team through case reviews to ensure continuity of care and coordination of services.
- Identifies quality improvement needs and participates in implementing improvements.
- Conducts ongoing in-service and orientation training for Munson Family Practice staff and the community.
- Maintains effective communications with patients, families, team members and managers as evidenced by mutual support, open communication, trust and support.
- Promotes good public relations with department personnel, physicians, other referral sources, patients, families and the community.
- Assumes responsibility for continuing education and professional development by attending required in-services and other means of continuing professional education (e.g., readings, classes, workshops).
- Adheres to all Ambulatory and department policies.
- Keeps manager and physician informed of activities, decisions and changes in patients’ treatment and/or progress.
- Leads team in multidisciplinary clinical problem-solving. Includes staff clinical supervision, conducting case conferences, staff meetings and other meetings as necessary.
- Coaches clinical staff to perform their responsibilities with efficiency and compassion through leading by example.
- Provides the Manager with input regarding competencies and annual discussions, and assists with implementation of hospital and departmental policies and procedures.
