Munson Home Health, in beautiful Traverse City, MI, is seeking an LPN to join our robust, comprehensive, multidisciplinary palliative care team. Two years of experience working with a specialty palliative care team preferred.

JOB SUMMARY
Provides clinical and/or administrative supervision in an Ambulatory Palliative Care Clinic.

ENTRY REQUIREMENTS
  • Graduation from a recognized school of nursing and a current Michigan LPN licensure, required.
  • Three years experience in a hospital or physician office setting, required.
  • CPR certification preferred or obtained within 90 days of hire and maintained.
  • Previous supervisory experience preferred.

Excellent interpersonal, communication and organizational skills. Must possess the ability to work independently while maintaining a team effort.
Able to effectively deal with conflict and conflict resolution.
Able to prioritize multiple tasks and duties and maintain a calm professional manner in a busy office setting.

ESSENTIAL DUTIES
  1. Strictly adheres to patient confidentiality.
  2. Directly coordinates the daily operations of the clinical functions under minimal supervision with written policies, procedures and processes as guidance.
  3. Maintains a professional working relationship with other hospital departments, patient’s families and other health care professionals.
  4. Assists as a liaison between other physician offices and hospitals to ensure patient needs are met.
  5. Assists in the planning and organizing of orientation and in-service training for staff members, and participates in guidance and educational programs.
  6. Acts as a role model maintaining a professional, customer focused demeanor at all times. Supports policy decision and implementation. 
  7. Acknowledges that safety is self-responsibility. Knows physical requirements of the job and works within those guidelines. Performs job duties safely at all times, utilizing learned body mechanics and transferring/lifting techniques. Plans actions to promote safety. Reports any unsafe situation/equipment according to hospital procedure. 
  8. Works within LPN or Medical Assistant scope of practice and performs job duties as outlined by Licensed Practical Nurse II and/or Medical Assistant job descriptions.
  9. Assists in the continuous Quality Improvement Processes in collaboration with other manager and other departmental staff.
  10. Assists in the ordering of clinical supplies, facility needs, and cost analysis of those items.
  11. Works to resolve problems and complaints regarding the office. Responds to provider requests regarding particular problem areas. 
  12. Assists manager with preparation for and compliance with inspections, including but not limited to joint commission, CMS, and Health Dept. 
  13. Assists in preparation of annual budget, making recommendations for equipment request and needs for the upcoming year. 
  14. Coaches support staff to perform their responsibilities with efficiency through leading by example and by working with support staff as necessary. Provides input to staff annual discussions. 
  15. Monitors attendance, overtime, PTO, and work utilization.
