Description
Munson Healthcare has the most comprehensive home health program in Northern Michigan. We are looking for team members who will continue our tradition of providing kind and respectful care for our patients in their homes.Graduation from a recognized school of nursing and a current Michigan RN licensure.
Registered nurses hired by Munson Home Health or transferred from Munson Medical Center to Munson Home Health prior to January 1, 2009 are encouraged to obtain a BSN. Certification in the specialty area of practice is also encouraged.
Registered nurses hired by Munson Home Health or transferred from Munson Medical Center to Munson Home Health on or after January 1, 2009, are required to obtain a BSN within ten years of their system hire date. Certification in the specialty area of practice is encouraged.
Home Health nursing experience preferred.
Infusion Nursing experienced preferred.
Demonstrated health assessment, organizational and interpersonal skills at an expert level.
Proven self-directed professional with ability to make independent judgments directing patient care.
Proven ability to communicate effectively in orally and in writing.
Possesses current Michigan motor vehicle license, ability to drive a car, and an insured vehicle capable of transport to patient homes or other offices in various weather conditions.
Meets MHH established requirements for proof of health status. Physically able to lift 35 pounds. Demonstrates ability to move, position and transfer clients utilizing good body mechanics, lifting techniques and/or assistive devices to avoid manipulating more than 35 pounds.
Meets Home Health and Medicare standards as evidenced by criminal background check and fingerprinting.
CPR certification required

Performs initial and ongoing comprehensive assessments of the client’s needs.
Initiates the plan of care and revises as necessary.
Provides those services and/or treatments requiring substantial and specialized nursing skill.
Counsels the client and family in meeting nursing and related needs.
Maintains clinical competency in IV Therapy.
Prepares clinical documentation, progress notes and required Daily Activity Record (DAR) as required by Munson Home Services documentation policy.
Refers to other services as needed.
Informs the physician and other staff of changes in the client’s needs or service requests.
Evaluates outcomes of care.
Participates in in-service programs and specialized training as may be required by referral sources.
Identifies problems and sets goals.
Consults with and educates the client and family regarding the disease process, self-care techniques and disease prevention strategies.
Requisitions and manages equipment and supplies appropriate to care.
Practices standard precautions.
Records in a timely manner evaluation data, interventions, and client’s response to interventions.
Participates in quality/performance improvement measures.
Employees are required to maintain proficiency in the basic functions of the e-mail program and are also required to regularly check email and keep calendars up to date.
Performs other duties and responsibilities as assigned.
