SUMMARY

 

Under the direction of the program manager, provides medical case management through linking, coordinating, monitoring, assessing and advocating for people living with HIV/AIDS and clients of the Thomas Judd Care Center. This will include, but is not limited to, obtaining medical care, nursing services, psychosocial opportunities, community interventions, legal referrals, financial determination for assistance and program eligibility, housing/utility assistance, and other necessary services for persons living with HIV/AIDS.

 

 ENTRY REQUIREMENTS

 

  • Associate degree in nursing with licensure current in the State of Michigan.  Bachelor’s degree in nursing must be completed per institution policy to maintain employment.
  • Certification as an HIV Case Manager and HIV Testing/Counseling through the State of Michigan. Must be completed within first year of hire and attendance at annual case management conference is required. 
  • Experience with persons living with HIV/AIDS and/or case management preferred, however, is not required.
  • Possesses a valid driver’s license as traveling/patient home visits are required.
  • Demonstrated oral and written communication skills with competency in Microsoft products.

  

ORGANIZATION

 

Works as a member of the HIV/AIDS case management team at the Thomas Judd Care Center under the direct supervision of the Manager of the program.   Reports to the Nurse Practitioner for medical supervision and direction.

 

SPECIFIC DUTIES

 

  1. Supports the Mission, Vision and Values of Munson Healthcare.

  1. Uses effective customer service/interpersonal skills at all times.

 

  1. Maintains MDHHS HIV case management/testing/counseling certificates through continuing education.

 

  1. Integrates HIV/STD prevention messages into education, support services activities, and client advocacy components of the program.

 

  1. Maintains HIV/AIDS competencies through ongoing specialized training related to working with people living with HIV/AIDS and their families.

 

  1. Under the direction of the program manager, provides medical case management through linking, coordinating, monitoring, assessing, and advocating for people living with HIV/AIDS and clients of the Thomas Judd Care Center. This will include, but is not limited to, intakes, early intervention services, providing PrEP medical services obtaining medical care, psychosocial opportunities, community interventions, legal referrals, financial determination for assistance and program eligibility, housing/utility assistance, and other necessary services for persons living with HIV/AIDS.

 

  1. Facilitates referrals to community programs to meet basic and/or individualized needs, obtain health care services, and/or obtain insurance coverage.

 

  1. Maintains client assessments, service plans, and progress notes in a timely manner.

 

  1. Completes third-party billing for client services in a timely manner.

 

  1. Documents HIV labs, medications, acuity levels, demographics, risk factors, HIV/AIDS diagnosis dates, case notes, and services in the MDCH database system to ensure program compliance with MDHHS Standards of Care.

 

  1. Confirms HIV diagnosis and identify related quality/care metrics to promote medical adherence.

 

  1. Identifies HIV primary care patients for readmission when discharged from the hospital.

 

  1. Assists NP with rooming, procedures, and sanitizing of exam rooms for both primary care and prevention services.

 

  1. Manages referrals to specialists for primary care patients.

 

  1. Transmit and schedule orders for diagnostics and procedures.

 

  1. Work comfortably with medically complex patients to improve health outcomes.

 

  1. Be willing to trained in phlebotomy and processing labs.

 

  1. Recognizes and respond appropriately to psychosocial risk factors.

 

  1. Meets the Principles and Standards of Services for HIV/AIDS client advocacy in Michigan.

 

  1. Plans and implements support services of the HIV program, including psychosocial functions, client transportation, and food pantry/supplemental food services.

 

  1. Authors articles for a client and provider newsletter on a regular basis.

 

  1. Participates in quality improvement plan activities and research studies as requested by the manager or nurse practitioner.

 

  1. Performs other duties and responsibilities as assigned.
