WHAT YOU WILL DO IN THIS ROLE:

As a Care Team Coordinator, you will be part of the care management team to ensure patients receive the care they need and that they understand their medical condition, medication(s), and other instructions.  The Care Team Coordinator is a liaison between the patient and the healthcare system.
 

WHAT WE ARE SEEKING IN YOU FOR THIS ROLE:

Credentialed Medical Assistant:

Must be a Credentialed Medical Assistant CMA, RMA, CCMA, NCMA, CMAC.

Certification with the American Association of Medical Assistants is preferred. 

BCLS/CPR Certification preferred.
 

Licensed Practical Nurse:

Graduate of an accredited LPN program. 

Current Practical Nurse license in the State of Michigan, in good standing.  BCLS/CPR certification. 

 

WHAT YOUR DAILY ACTIVITES AND EXPECTATION WILL BE IN THIS ROLE:

Help identify patients eligible for care management services utilizing patient high risk registry list, ER list, ADT list, PCP referral etc.                                 

Helping create and promote patient adherence to care plans.  This will entail follow up with patient, patient families, members of the care team (i.e. pharmacy, social work, specialist etc.) 

Help to increase continuity of care for patient, set agendas and manage future visits.

Help increase patient’s ability for self- management promoting patient’s health.

Assist with transitional care calls, medication reconciliation and documentation.

Help assess patient’s unmet health and social needs, connect patient and patient families to relevant community resources.

Contact patients with identified gaps in care as needed for ongoing health maintenance.

Provide additional resources/community linkages to under insured patients.

Facilitate transitions of care with the practice team members ensuring timely follow up services and care coordination

Pushing Care Management billing codes and keeping track of incentive percentages needed.  

Clinical duties (may include, but not limited to):      

Taking medical histories

Explaining treatment and procedures to patients

Timely follow through and completion of referral requests

Preparing patients for examination

Assisting the physician during exams

Collecting and preparing lab specimens

Preforming basic laboratory tests including blood draws

Instructing patients via standardized pathways and protocols

Preparing and administrating medications as directed by the physician. 

Injections

Removing sutures and changing dressings

Other clinical services as required by specialty. 

Administrative duties (may include, but not limited to):

Answering phones

Greeting patients

Documenting and updating pertinent medical record requirements

Coding and filling out insurance forms

Scheduling appointments

Arranging for hospital admissions, referrals, and laboratory services.

Using computer applications
