ENTRY REQUIREMENTS
Graduation from a recognized school of nursing and a current Michigan RN licensure.
Acute care, home care, or hospice experience preferred.
Registered nurses hired by Munson Home Health or transferred from Munson Medical Center to Munson Home Health prior to January 1, 2009, are encouraged to obtain a BSN. Certification in the specialty area of practice is also encouraged.
Registered nurses hired by Munson Home Health or transferred from Munson Medical Center to Munson Home Health on or after January 1, 2009, are required to obtain a BSN within ten years of their system hire date. Certification in the specialty area of practice is encouraged.
Proven ability to communicate effectively in verbal and written English. Computer literacy required.
Proven health assessment, organizational, and interpersonal skills with the ability to make independent judgments directing patient care.
Meets Home Health and Medicare standards as evidenced by criminal background check and fingerprinting.
Possesses current Michigan motor vehicle license, ability to drive a car, and an insured vehicle capable of transport to patient homes or other offices in various weather conditions.
Meets MHH established requirements for proof of health status. Physically able to lift 35 pounds. Demonstrates ability to move, position and transfer clients utilizing good body mechanics, lifting techniques and/or transfer assistive devices to avoid manipulating more than 35 pounds.
CPR certification preferred.
ORGANIZATION
Reports to the manager, clinical supervisor or designee.
Provides direction to and supervision of Home Health Aides, LPN’s and homemakers.
Interacts interdependently with other Home Health staff and volunteers, physicians, support staff, other hospital staff and members of the community.
SPECIFIC DUTIES

Uses effective customer service/interpersonal skills at all times.
Meets established productivity standards.
Uses established program specific criteria to determine eligibility for certification/ recertification and placement in the appropriate level of care.
Uses a framework for assessment, implementation, coordination and evaluation to provide outcome-based services for an assigned patient caseload from admission through discharge.
Performs initial and ongoing comprehensive assessments of the client’s needs, including Outcome and Assessment Information Set (OASIS) assessments when required.
Initiates the plan of care, identifying problems, setting goals, and plan for discharge. Revises goals and discharge planning as necessary through evaluation of outcomes.
Provides those services and/or treatments requiring substantial and specialized nursing skill, including preventative, rehabilitative, palliative and end of life nursing care.
Counsels the client and family in meeting nursing and related needs.
Initiates appropriate preventive and rehabilitative nursing procedures.
Coordinates and refers services including, but not limited to, Home Health Aide and Occupational Therapy, Physical Therapy, Speech Therapy and Social Work, Chaplain, and Bereavement Care to specific patients, as appropriate.
Supervises LPNs and paraprofessionals providing services.
Participates in in-service programs.
Informs the physician (where applicable) and other team members of changes in the client’s needs, condition, or service requests by using Munson Home Health’s current processes and technology.
Consults with and educates the patient and family regarding the disease process, self-care techniques, disease/symptom management, palliative and end of life care, as appropriate.
Secures equipment and supplies required based on plan of care, including pharmaceuticals for Hospice patients.
Evaluates own needs and seeks personal support and/or guidance of management or another member of the interdisciplinary team, as needed.
Records in a timely manner evaluation data, the plan of care, interventions, client’s response to interventions, and all other clinical patient documentation in the electronic medical record.
Participates in quality/performance improvement measures.
Employees with e-mail are required to maintain proficiency in the basic functions of the program and are also required to regularly check email and keep calendars up to date.
